Fertility Massage Before IVF or IUI: When Should You Schedule It?

Preparing for IVF or IUI can feel like your life suddenly revolves around calendars.

Cycle day one. Bloodwork. Ultrasounds. Medications. Injections. Follicle checks. Retrieval. Insemination. Transfer.

And somewhere in the middle of all of that, many women begin searching for things they can do to feel more supported physically and emotionally during the process.

One question we hear at Rebirth Maya Massage is:

The simplest answer is that deeper abdominal and pelvic fertility bodywork is generally best completed before active ovarian stimulation or before your fertility clinic reaches the stage when they want you limiting abdominal pressure and physical activity.


At Rebirth Maya Massage in San Diego, we work with women preparing for natural conception as well as clients undergoing IVF and IUI. Our fertility-focused services include Mercier Therapy and Mayan abdominal/womb massage, and timing is customized around your individual fertility treatment plan.

What Is Fertility Massage?

“Fertility massage” is a broad term used for external bodywork intended to support the abdomen, pelvis, surrounding muscles, connective tissues, relaxation, and overall wellbeing before conception.

At Rebirth, fertility clients may receive:

  • Mercier Therapy

  • Mayan abdominal or womb massage

  • Massage to the abdomen and pelvis

  • Low-back and sacral bodywork

  • Hip and pelvic muscle work

  • Relaxation-focused massage

  • Other supportive therapies when appropriate

All of this work is external.

Fertility massage is not an internal pelvic treatment, and it does not replace medical fertility evaluation or treatment.

Most importantly, fertility massage should not be marketed as a guarantee that someone will become pregnant.

The American Society for Reproductive Medicine (ASRM) has reviewed massage around IVF embryo transfer and concluded that there is currently insufficient evidence to recommend massage as a way to improve IVF pregnancy or live-birth rates.

That does not mean massage cannot be valuable during a fertility journey. It means we need to be clear about what it can—and cannot—promise.

Why Do Women Seek Fertility Massage Before IVF or IUI?

Going through fertility treatment can be physically and emotionally demanding.

Women may seek fertility-focused bodywork because they want:

  • A dedicated time to relax

  • Support for abdominal or pelvic tension

  • Relief from low-back and hip tightness

  • A greater sense of connection with their bodies

  • Support before beginning fertility medications

  • A complementary wellness ritual during an otherwise highly medical process

Some women also come to us after previous abdominal or pelvic surgery, while managing conditions such as endometriosis or fibroids, or after months or years of trying to conceive.

Every fertility story is different.

That is why your massage plan should be based on your history and where you currently are in your medical fertility cycle.

What Is Mercier Therapy?

Mercier Therapy is a structured form of external pelvic manual therapy that focuses specifically on the pelvic region.

At Rebirth, our Mercier Therapy protocol consists of six one-hour sessions completed within approximately six weeks. Rebirth offers Mercier Therapy independently or as preparation before medically assisted fertility cycles such as IUI or IVF.

The techniques are intended to work with pelvic tissue mobility and surrounding restrictions using external manual therapy.

Mercier Therapy differs from a general relaxation massage because the sessions are much more targeted to the pelvis.

It also differs somewhat from our traditional Mayan abdominal massage, which takes a more holistic approach and may incorporate additional areas of the body.

What Is Mayan Womb Massage for Fertility?

Mayan abdominal massage—often called womb massage or sobada de matriz—is another form of external abdominal and pelvic bodywork.

Sessions may focus on:

  • Abdomen

  • Low back

  • Sacrum

  • Hips

  • Surrounding soft tissue

  • Overall relaxation

At Rebirth, we are trained in both Mayan abdominal massage and Mercier Therapy, which allows us to recommend the approach that best fits a client's goals and history rather than giving every fertility client the exact same treatment.

When Is the Best Time to Start Fertility Massage Before IVF?

Ideally, start before your IVF stimulation cycle begins.

If you know that IVF is several weeks or several months away, that gives you the most flexibility.

Women choosing the full Mercier Therapy protocol should allow enough time to complete the six sessions before beginning ovarian stimulation whenever possible.

A simple planning example might look like:

IVF begins in 6–8 weeks: This may provide enough time to complete the full Mercier Therapy series.

IVF begins in 3–4 weeks: You may still be able to receive fertility-focused bodywork, but the plan may need to be condensed or modified.

IVF medications have already begun: Contact us before booking abdominal work. Your treatment plan needs to be coordinated around your fertility clinic's instructions and your stage of stimulation.

Egg retrieval is approaching: Deeper abdominal or pelvic massage is generally not the time to experiment with.

After embryo transfer: We only offer relaxation massage at this point.

Why Does Timing Matter During IVF?

During an IVF cycle, medications stimulate the ovaries to develop multiple follicles at the same time.

This means the ovaries can become significantly larger and more sensitive than they are during a normal menstrual cycle.

UCSF explains that IVF begins with ovarian stimulation to produce multiple eggs before retrieval.

NYU Langone's fertility guidance significantly restricts exercise later in ovarian stimulation and through the period after egg retrieval because enlarged ovaries have a greater risk of ovarian torsion, in which an ovary twists.

For that reason, deep abdominal and pelvic work should not simply continue unchanged throughout IVF stimulation.

Your fertility doctor's instructions always take priority.

If you have started injections, tell your massage therapist:

  • What medications you are taking

  • Which day of stimulation you are on

  • Your expected retrieval date

  • Whether your ovaries are enlarged

  • Whether you have significant bloating or discomfort

  • Any restrictions your fertility clinic has given you

When in doubt, we may ask you to obtain approval from your fertility clinic before proceeding.


Can I Get Fertility Massage While Taking IVF Medications?

Possibly—but the type of massage matters.

A relaxing massage to areas such as the:

  • Neck

  • Shoulders

  • Upper back

  • Arms

  • Hands

  • Feet

is very different from deep manual therapy directly over stimulated ovaries and the lower abdomen.

Once ovarian stimulation is underway, we become much more cautious about abdominal and pelvic techniques.

This is why we strongly encourage women to begin fertility-focused abdominal work before stimulation whenever possible.

You can still receive supportive massage during many parts of your fertility journey, but the session may shift away from deep pelvic bodywork.

When Should Fertility Massage Stop Before Egg Retrieval?

There is no universal medical rule stating that every massage therapist must stop on one exact IVF cycle day.

IVF protocols vary substantially from patient to patient.

However, once the ovaries are becoming enlarged from stimulation, deep abdominal and pelvic massage should generally be avoided unless your reproductive endocrinologist has specifically cleared it.

Some fertility centers begin restricting physical activity within the first several days of stimulation because the ovaries are growing and torsion risk rises.

This is one of the reasons we prefer to complete more intensive fertility bodywork before the stimulation phase rather than trying to squeeze sessions in immediately before retrieval.


What About Massage Before IUI?

IUI is different from IVF, but timing still matters.

Some IUI cycles use medications that stimulate one or more follicles. UCSF notes that fertility medications used for ovulation induction may be used with both IUI and IVF and that ovarian response needs to be individually monitored.

If you are preparing for an IUI, tell us:

  • Whether you are doing a natural or medicated cycle

  • What fertility medication you are taking

  • Your expected ovulation or trigger date

  • Your planned insemination date

  • Any instructions from your fertility provider

When possible, fertility-focused abdominal work is usually easiest to plan earlier in the cycle and before ovulation/insemination.

Once conception may have occurred, we stop treating the body as though pregnancy is impossible.


Can I Get Womb Massage After My IUI?


Once you have undergone an IUI and are in the two-week wait, we recommend being conservative with deeper uterine or pelvic work.

You are now in a window where pregnancy is possible.

Instead of deeper fertility abdominal massage, supportive care may shift toward relaxation-focused massage that avoids aggressive abdominal techniques.

If pregnancy is confirmed, womb massage and prenatal massage become two completely different services.

Mayan uterine massage and Mercier Therapy are not continued as standard fertility treatments during pregnancy.

You would instead transition to pregnancy-appropriate care.

Can I Get Massage Before Embryo Transfer?


This is an area where careful wording matters.

Massage may be relaxing before an embryo transfer, but there is not strong evidence showing that massage increases IVF success.

ASRM reviewed massage before embryo transfer and found insufficient evidence to recommend for or against it as a method for improving IVF outcomes.

So if your goal is relaxation, a gentle massage may be something to discuss with your fertility provider.

But you should be skeptical of anyone promising that a particular massage immediately before transfer will make an embryo implant.

No ethical massage therapist can guarantee implantation.


What About Massage After Embryo Transfer?


After an embryo transfer, we take a more conservative approach.

You do not need to lie completely still after transfer—ASRM actually supports immediate ambulation after embryo transfer—but this does not mean that deep abdominal massage is necessary or beneficial.

At this stage:

  • Follow your fertility clinic's instructions

  • Avoid deeper uterine or pelvic manipulation

  • Tell any massage therapist that you have had an embryo transfer

  • Treat yourself as potentially pregnant

  • Ask your fertility team if you are unsure about a specific therapy

If you would like massage purely for relaxation during the waiting period, we can discuss pregnancy-appropriate modifications.


What About Frozen Embryo Transfer?


A frozen embryo transfer cycle may look very different from a fresh IVF cycle because your ovaries may not be undergoing the same degree of stimulation immediately before transfer.

That does not mean every abdominal treatment is automatically appropriate.

Medication schedules and protocols vary widely.

If you are preparing for a frozen embryo transfer, let us know:

  • Your transfer date

  • Whether you are taking estrogen or progesterone

  • Whether you recently completed an egg retrieval

  • Whether your fertility doctor has placed any restrictions on you

A customized plan is always safer than assuming all IVF cycles are alike.


Can Fertility Massage Increase Blood Flow to the Uterus?


You may see this claim frequently online.

Massage naturally influences superficial tissues and circulation in the areas being worked, and manual therapies are often used with the goal of encouraging tissue mobility and relaxation.

However, saying that fertility massage increases uterine blood flow enough to improve implantation or pregnancy rates goes beyond what current high-quality research can establish.

That distinction matters.

We can support your body and your overall wellbeing without promising that massage will alter your IVF outcome.

Can Fertility Massage Break Up Scar Tissue?


This is another claim that deserves nuance.

Manual therapy can influence the mobility of skin, fascia, muscles, and some superficial scar tissue.

But massage cannot simply “break apart” every internal adhesion in the abdomen or pelvis.

Significant internal adhesions may require medical assessment, particularly when they are related to:

  • Endometriosis

  • Pelvic surgery

  • C-section

  • Myomectomy

  • Infection

  • Other gynecologic procedures

Fertility massage should be viewed as complementary manual therapy—not a substitute for medical diagnosis or surgery when those are needed.


Fertility Massage for Endometriosis

Many women going through IVF also have endometriosis.

Fertility bodywork may be used to support relaxation and muscular comfort, but it does not remove endometriosis lesions or cure endometriosis.

At Rebirth, Mercier Therapy is not recommended during menstruation for clients with endometriosis; sessions are instead scheduled after the period has ended.

If your endometriosis causes severe pelvic pain or your fertility doctor has given you activity restrictions, let us know before your appointment.


Fertility Massage for Fibroids

Women with uterine fibroids also frequently find us while preparing for pregnancy.

Massage cannot shrink or eliminate fibroids.

However, a woman with fibroids may still receive appropriately modified bodywork depending on:

  • Fibroid size

  • Location

  • Symptoms

  • Bleeding

  • Tenderness

  • Fertility treatment plan

  • Medical guidance

Very large, painful, or symptomatic fibroids require individualized care.


Is Fertility Massage Painful?


It should not be unbearably painful.

Mercier Therapy is more focused and may involve deeper pressure than a relaxation massage, but you should always be able to communicate with your therapist.

If pressure feels sharp, frightening, or excessively painful, it should be adjusted.

There is no fertility benefit to forcing your body through extreme pain.

At Rebirth, we consider your medical history, previous surgeries, menstrual cycle, fertility treatment timeline, and comfort when deciding how to approach the session.

Do I Need My Fertility Doctor's Permission?

If you are not yet taking fertility medications and do not have significant medical complications, formal written clearance may not always be necessary.

Once you are actively undergoing IVF or medicated IUI, however, communication with your fertility provider becomes much more important.

We recommend checking with your fertility doctor when:

  • You have started ovarian stimulation

  • Your ovaries are enlarged

  • You are close to egg retrieval

  • You recently underwent retrieval

  • You have symptoms of ovarian hyperstimulation syndrome

  • You have severe pelvic pain

  • You have recently undergone embryo transfer

  • Your fertility clinic has restricted physical activity

  • You are unsure whether pregnancy is possible

Your reproductive endocrinologist knows exactly how your ovaries are responding to medication.

Their guidance takes priority over a general massage schedule.


Symptoms That Need Your Fertility Clinic—Not a Massage Appointment


Contact your fertility clinic promptly if you experience concerning symptoms during treatment, particularly after ovarian stimulation or retrieval.

These can include:

  • Significant or rapidly increasing abdominal swelling

  • Severe pelvic or abdominal pain

  • Difficulty breathing

  • Persistent vomiting

  • Rapid weight gain associated with fluid retention

  • Very low urine output

  • Faintness

  • Significant one-sided pelvic pain

  • Any symptom your clinic has told you to report

Ovarian stimulation can rarely cause ovarian hyperstimulation syndrome (OHSS) and other complications. Fertility medications can enlarge the ovaries, and monitoring is an essential part of treatment.

Massage is not an appropriate treatment for these complications.


How Far in Advance of IVF Should I Start Mercier Therapy?


If you want to complete the full Rebirth Mercier Therapy protocol, give yourself approximately six weeks before stimulation whenever possible.

Our protocol includes six one-hour sessions completed within a six-week period.

That makes an ideal planning window something like:

6–8+ weeks before IVF

Excellent time to schedule your complimentary evaluation and begin a complete series if appropriate.

3–5 weeks before IVF

Contact us. We may still be able to create an individualized plan before stimulation begins.

Already taking stimulation medication

Do not automatically book deep abdominal therapy. Schedule an evaluation so we can review your cycle and determine what type of bodywork is appropriate.

Approaching retrieval

Follow your fertility clinic's restrictions. Deep abdominal/pelvic therapy is generally not our focus at this stage.

After transfer

Transition away from fertility-focused uterine work and follow your fertility provider's guidance regarding massage.

Planning ahead gives you far more flexibility.

The safest approach is communication between you, your fertility team, and your massage therapist.
